Your forms, gone digital. The answers straight into your system.
Patients fill in your own forms on their own phone and sign with a finger. The answers reach your system as data, along with a PDF in the familiar layout if you need one. Paper only when you want it.
Key Features:
- ✓The answers as data in your system
- ✓Your familiar form, 1:1
- ✓Or no paper at all
- ✓Signed on the phone
- ✓The patient's own phone
- ✓Only your practice can read it
A paper form gets written twice.
Once by the patient, with a pen on a clipboard. And once more at the front desk, when someone deciphers the handwriting and types it into the practice software, while the next patients are already waiting.
The second time can go.
Which practices this suits
This is for you if…
- Your patients fill in paper forms before their appointment
- Someone at the front desk types the answers in and scans the sheets
- You want to keep your own forms rather than rebuild them in a form builder
- You want to work without paper, or keep filing the familiar sheet
This is not for you if…
- You already have an online form you are happy with
- You want patients to fill in a publisher's consent forms digitally
- Your software must take the data over GDT today
- You want to build and change your forms yourself
What your practice gets
The answers as data in your system
Typed, not handwritten, each answer under a fixed key. Nothing to retype, no handwriting to decipher.
Your familiar form, 1:1
No rebuild in someone else's form builder: the patient fills in your own form, and it also arrives as a PDF in the familiar layout, to file or to print.
Or no paper at all
If you don't need the PDF, work with the data alone. Paper is then only for when you or a patient want it.
Signed on the phone
With a finger, in the place where the signature goes on paper.
The patient's own phone
Scan the QR code, fill in, send. No app, no account, and no tablet at the front desk to charge and disinfect.
Only your practice can read it
Encrypted in the patient's browser with your practice's key. Our server sees only encrypted data and holds it for 60 minutes at most.
How it works
From your practice's form to the data in your system, without a new device at the front desk.
You give me your forms
As a PDF or a scan. I set up every field once; your form stays as it is. If you like, entry questions decide up front which forms the patient gets.
The patient scans the QR code
From the poster in the waiting room or the flyer at the desk. Your form opens in the browser of their own phone.
Fill in and sign
One question at a time; whatever is already on one form is filled in on the next. Then a signature with a finger, and „An die Praxis senden“ (send to the practice).
The data arrives with you
Encrypted, as data and as a PDF. Your software fetches it, or we send it to an address of your system. How exactly, we set up for your software.
What it costs
Monthly by the number of sendings, not by the number of doctors. Unlimited staff, no devices.
S
€29 a month
up to 150 sendings a month
M
€69 a month
up to 500 sendings a month
L
€129 a month
up to 1,500 sendings a month
A sending is one set of forms a patient sends with one tap, the short sign-in of an already known patient included.
On top
- Beyond the package, €0.20 per sending. Nobody is turned away.
- Setup, once, €129: 2 forms and up to 2 entry questions with 3 paths that decide which forms the patient gets.
- Every further form, €25 per page.
- A further location with its own forms and QR code, €15 a month; the package stays shared.
- Under your own name and domain, with no mention of A4Norm after sending: €79 a month on top of any tier, setup from €690.
Prices are final: no VAT is charged — small business under § 19(1) of the German VAT Act (UStG). Introductory prices as of October 1, 2026. Prices may change; should the small-business rule no longer apply, VAT will be added.
Who can read the answers
- Encrypted on the patient's phone, in the browser, with your practice's key. Only you hold the key to decrypt it.
- Our server cannot read the answers. It holds only the encrypted package until your system picks it up, 60 minutes at most, and deletes it afterwards.
- No IP addresses: we store no IP address with a sending.
- Three hours on the phone: that is how long the draft stays there for corrections; then it is deleted.
- Servers in Germany, and a data processing agreement (AVV) with every practice.
Frequently asked questions
Do patients need an app?
No. The QR code opens your form in the phone's browser. No account, nothing to install.
And someone without a smartphone, or who prefers paper?
Gets paper, as before. The phone is an extra way, not an obligation.
Which forms work?
Your own, as a PDF or a scan. I set them up; you have nothing to rebuild. Consent forms from a publisher are usually licensed; whether they may be filled in digitally is the publisher's decision.
How does the data get into our software?
One of two ways: your software asks for new sendings itself, over an outgoing connection, or we send each sending, signed, to an address of your system and try again if it does not answer. A sending holds the answers under fixed keys (name, date of birth and so on, the same on every form), the same answers as a FHIR QuestionnaireResponse, and the filled-in PDF. There is no ready-made GDT connection yet; what your software needs, we work out in a conversation.
Does the app assess the answers?
No. The answers reach you exactly as the patient entered them. What follows from them is your call.
What does the patient see after sending?
The status: „Gesendet“ (sent), then „Von der Praxis abgeholt“ (picked up by the practice). Below it, a short, optional question whether this was easier than paper, and a note about A4Norm, the free scanner for their own papers. Anyone who says they run a practice or business of their own also sees a short note about A4Norm Check-in. With White Label, both are gone.
Can the patient still correct an answer?
Yes, for three hours. They send again, and a new version of the same sending reaches you.
Does this work outside medicine?
Yes, wherever clients fill in a form before their first appointment: therapy practices, law offices, studios or veterinary practices. The way is the same, and the form stays yours.
Bring one of your forms to the demo
I'll show you the whole way from the QR code to the data in your system, and on your form we'll see right away what setting it up takes.
